Gout | Causes, Pathophysiology, Risk Factors, Symptoms, Diagnosis, Treatment

Gout is a crystal inflammatory monoarthropathy caused by deposition of monosodium urate crystals into joints and other areas of the body. In this lesson, we discuss the pathophysiology as to how crystals are formed and how inflammation occurs in gout. We also discuss many risk factors for getting gout, triggers of gout flares, signs & symptoms, how gout is diagnosed and how it is treated.

Cushing syndrome is a group of conditions caused by high levels of circulating cortisol or related corticosteroids.
A gout attack occurs suddenly, often at night. The affected joint is swollen, red and warm to touch. The initial attack usually involves only one joint, most often in the big toe, and lasts a few days. Subsequent flares may involve multiple joints and can last for weeks, with shorter intervals of remission in between.
Gout is caused by deposits of urate crystals in the joints, which may happen when blood levels of uric acid are too high. Uric acid is a waste product from the breakdown of purines, a major component of DNA and RNA. Because body’s cells are constantly renewed, uric acid is constantly produced and removed in urine. High serum urate level may result from decreased urate excretion, increased urate production, and/or overconsumption of purines.
The most common cause is the kidney’s decreased ability to excrete uric acid, which can be hereditary, or a result of kidney diseases, medications such as diuretics, alcohol use, or lead poisoning. Increased production of urate may occur in conditions with abnormally high cellular turnover or cell death, or in obesity, as urate production increases with greater body surface area. Overconsumption of purinerich foods can contribute to higher urate level, but is rarely the only cause.
High concentration of urate promotes its precipitation and formation of crystals in joints, causing inflammation and pain. Because uric acid crystals form more easily in cooler temperatures, gout more often affects joints of the extremities. Urate crystals may also deposit under the skin around the joints, forming hard lumps, called tophi, that are not usually painful but can limit joint motions and lead to deformities. Joint damage caused by gout also increases the risk of developing osteoarthritis. In the kidneys, urate deposits may form stones, blocking urine flow.
Gout is more common in men. It often starts in middle age in men, and after menopause in women. Other risk factors include diets, obesity, other health conditions, family history, and surgery or trauma.
Diagnosis requires detection of urate crystals in synovial joint fluid. Because not all people with high serum urate levels develop gout, a blood test alone is not sufficient for diagnosis.
Acute gout attacks are treated with antiinflammatory drugs, such as NSAIDs, regular use of which may also help prevent or reduce frequency of attacks. Because tophi can be dissolved by lowering serum urate, medications that block urate production or increase urate excretion may be used for this purpose.

GOUT, Causes, Signs and Symptoms, Diagnosis and Treatment.

Gout is a form of inflammatory arthritis characterized by recurrent attacks of a red, tender, hot, and swollen joint. Pain typically comes on rapidly, reaching maximal intensity in less than twelve hours. The joint at the base of the big toe is affected in about half of cases. It may also result in tophi, kidney stones, or urate nephropathy.

Gout is due to persistently elevated levels of uric acid in the blood. This occurs from a combination of diet, other health problems, and genetic factors. At high levels, uric acid crystallizes and the crystals deposit in joints, tendons, and surrounding tissues, resulting in an attack of gout. Gout occurs more commonly in those who regularly eat meat or seafood, drink beer, or are overweight.Diagnosis of gout may be confirmed by the presence of crystals in the joint fluid or in a deposit outside the joint. Blood uric acid levels may be normal during an attack.

Treatment with nonsteroidal antiinflammatory drugs (NSAIDs), steroids, or colchicine improves symptoms. Once the acute attack subsides, levels of uric acid can be lowered via lifestyle changes and in those with frequent attacks, allopurinol or probenecid provides longterm prevention.Taking vitamin C and eating a diet high in lowfat dairy products may be preventive.

Best & Worst Foods to Eat with Gout | Reduce Risk of Gout Attacks and Hyperuricemia

Best & Worst Foods to Eat with Gout | Reduce Risk of Gout Attacks and Hyperuricemia

Gout is an inflammatory monoarticular arthropathy that is caused by monosodium urate crystal deposition within particular joints in the body. The urate crystals result from hyperuricemia (high levels of uric acid in the blood), which is derived from purine metabolism. Purines can come from a variety of different foods, with some dietary selections having high purine content, and other selections having lower amounts of purines. In this lesson, we discuss the worst foods to eat with gout (foods and beverages that have high levels of purines), and the best foods to eat with gout (foods and beverages that have lower levels of purines).

  1. If you ask me, the keto diet is not for most people. It's just really tough to adhere to and you feel exhausted. But the Agoge diet on the other hand feels great, and it works too.

  2. Man sucks all the really good foods is what triggers my gout. I thought dairy products increased your chance of gout tho? Didn't know it decreased it, my dad and brother would always get mad when and say that when I would chug a glass of milk with gout.

  3. this video was a great help to me as my sister was suffering from the same disease and with the help of the excellent medicine provided by Planet Ayurveda my sister disease was cured

  4. As someone who has suffered from bad gout attacks, I will put my two cents in. I eat whatever I want, Drink to much, but I am working on losing my fat lol. Anyway I found drinking lemon water to stop 95 percent of attacks, no matter what I eat or drink. I don't even drink lemon water every day anymore but when I feel an attack coming I go for lemon water. Also increase your water intake by 3 times. I could not handle not eating all these great foods . Dam I also stopped drinking beer, its vodka now lol. Cherries never helped my gout at all, just saying.

  5. My belly fat is so stubborn that I tried the keto diet, saw some results but felt terrible. The Agoge diet on the other hand I saw the same results but felt great.

  6. I like your diet the best. Of course no one seems to know conclusively. Many I have seen, in addition to meat, shellfish, alcohol, high fructose corn syrup, also remove mushrooms, beans, legumes, hi fat dairy, strawberries oranges and some other fruits, yeast and whole grains, peanuts. Which doesn’t leave much to eat.

  7. I adjusted my diet big because of Gout. Went to a Cardiologist and we discussed my Gout. He asked me what I was doing “I said no Shellfish, no Soda, no Caffeine, much less red meats”. He said “why would you want to live without Shrimp and Crab”? And prescribed me 100 mg of Allopurinol twice a day. Never had a flare up since. It’s been about twelve years. I do eat Cherries like a Maniac when they’re in season, Strawberries and Blueberries too. . Now I’m juicing. Beets, Carrots, Red Cabbage, Ginger, Apples etc. Survived a big bout with the ‘Vid. At 65 years old with Emphysema. Caught it at my Endocrinologist’s office. (Thyroid) Cheers.

  8. Can't disagree more with the recommendation of soy products. I had my first gout attack after eating a Chinese dish with tofu. I got it by mistake, as it was late at night and I was hungry (they screwed up my order as usual). I wouldn't normally eat tofu, and I believe it brought on my first gout attack. Have had a few more since then, and cherries really knocked it right out of me during one attack.

  9. thanks for this info. some doctors have nuts and legumes on their food-to-avoid list. some sources say they're good for gout. just confused on that part.
