Gout is an inflammatory monoarticular arthropathy that is caused by monosodium urate crystal deposition within particular joints in the body. The urate crystals result from hyperuricemia (high levels of uric acid in the blood), which is derived from purine metabolism. Purines can come from a variety of different foods, with some dietary selections having high purine content, and other selections having lower amounts of purines. In this lesson, we discuss the worst foods to eat with gout (foods and beverages that have high levels of purines), and the best foods to eat with gout (foods and beverages that have lower levels of purines).

Gout Attack Symptoms, Treatment, Diet, Pharmacology Nursing Lecture for NCLEX Explained


Gout attack (acute or chronic) nursing lecture on the symptoms, treatment, diet, and pharmacology review for the NCLEX exam.

Gout is a type of arthritis due to the accumulation of uric acid in the blood. Symptoms of gout are due to uric acid crystals that form within the joint that leads to severe pain, redness, and limited mobility.

Acute gout attacks happen suddenly (often waking the patient in the middle of the night) and tends to affect the big toe (knee, elbow, fingers, toe can also be affected). The affected joint will present with swelling, inflammation, and severe pain. The site will also be warm to the touch, and the patient will be unable to tolerate even the slightest pressure on the site.

Chronic gout is due to constant elevated uric acid levels that leads to repeated acute gout attacks. In chronic gout, tophi tend to develop. Tophi are large masses of uric acid crystals that have formed into large clumps. They are white/yellowish nodules that can found under the skin (helix of the ear, fingers, elbow etc.), within the joints, or bones. Tophi can lead to bone deformity and joint damage.

Patients with gout are educated to follow a diet that is low in purines, alcohol, and high fructose corn syrup drinks. Food high in purines include seafood, organ meats, red meat, and alcohol (especially beer).

Nursing interventions for gout include: keeping the patient hydrated with 23 liter of fluid per day (prevent renal stones and keep uric acid level low), monitor joint’s status of redness and swelling, cold/warm compresses as tolerated, administering antigout medications.

  3. Wait did you really just say that gout has nothing to do with uric acid levels? I'm not a doctor or a medical student. Just someone with a VERY HURTY TOE who has read everywhere else that high uric acid levels is the cause of gout

  4. Very informative, humorous – I enjoyed thoroughly.
    I'm a 71 YO with a 30 year gout history. For first several years I followed MD's orders, took prescribed drugs, stayed away from numerous foods/drinks. The results were not good so I stopped the MD and the prescription drugs.
    Began regimen of natural items: Started w/dark cherry juice and ACV. Over time added turmeric, fresh lemon juice/pulp. cinnamon. black pepper, celery seed extract, garlic along with staying away from high purine foods/drinks. Results better than MD and pharma drugs but still had attacks.
    Finally went to lowcarb/highfat and keto diet. Results have been excellent – pain and swelling gone first week. Although not fat or hypertension, weight has dropped several pounds and blood pressure reduced. Sleep is better.
    I believe that SUGAR was the bad guy BUT, I enjoy the lowcarb/highfat keto diet so I will continue on.
    I also dislike any type of drugs so I am happy to be away from them.
