This weeks video idea came to me after a patient asked me what they can do, to help prevent gout attacks.

Gout is in fact one of the most common forms of arthritis. We all know someone who suffers from gout so please help spread the word on this video so more people can learn how to prevent an attack.


Gout is caused by having too much of the chemical, uric acid, in your bloodstream. Uric acid is the waste product created when the body breaks down purines (a type of protein found in many foods and all of your cells).

Most of the uric acid is passed out with the urine and some from the gut with the stools (faeces). In people with gout the amount of uric acid in the blood builds up. From time to time the level may become too high and tiny gritlike crystals of uric acid may form. The crystals typically collect in a joint. The crystals irritate the tissues in the joint to cause inflammation, swelling and pain a gout attack.


Lifestyle factors may reduce the risk of having gout attacks. These include losing weight (if overweight), eating a healthy diet and not drinking much alcohol or sugarsweetened soft drinks. Feel free to watch my video for more detail.


Any joint can be affected by gout, but it usually affects joints towards the ends of the limbs, such as the toes, ankles, knees and fingers. Signs and symptoms of gout include:

• Severe pain in one or more joints
• The joint feeling hot and very tender
• Swelling in and around the affected joint
• Red, shiny skin over the affected joint

Symptoms develop rapidly over a few hours and typically last three to 10 days. After this time the pain should pass and the joint should return to normal.

Almost everyone with gout will experience further attacks at some point, usually within a year.

See your GP if you suspect you have gout and it hasn’t been previously diagnosed, particularly if the pain keeps getting worse and you also have a high temperature (fever).

It’s important that a diagnosis is confirmed because other conditions that require urgent treatment, such as an infected joint, can sometimes cause similar symptoms.

David Wells, the lefthanded pitcher who has played for the New York Yankees, the Toronto Blue Jays has described his first gout attack. He said that he just kind of woke up one morning, started walking and took one stride, and then he started to scream in a high falsetto tone like a sixyearold girl. Then he states that he fell to the ground and started grabbing his left, big, painful toe.

What can make a strong star athlete fall to the ground and scream in pain? Gout can do it. What is gout? Gout is a type of arthritis or joint inflammation. It occurs in some people who have elevated levels of urate, or uric acid. Now, just because you have elevated levels of uric acid, that does not necessarily mean that you will have gout. In fact, twothirds of people with elevated uric acid have no gout. But in the patients who do, this uric acid forms into crystals that deposit in the tissues and in the joints.

Now with my patients who have these crystals in the joints, some of them have described it like having shards of glass inside your joint stabbing. When these uric acid crystals form in the joints, they trigger an inflammatory reaction which causes pain, swelling, and redness of the joint. Now, gout can occur in any joint, but it very typically can present in the big toe, which is what David Wells described.
What are the risk factors for gout or symptoms of gout? Chronic kidney disease is a risk factor, which explains why I have so many patients with gout. Also, hypertension or high blood pressure is a risk factor. Obesity, taking certain medications that increase the levels of uric acid in the blood such as diuretics can be a risk factor, consuming excessive amounts of alcohol like beer, wine, also a risk factor, and consuming excessive amounts of meats and seafood as well as nondiet sodas with high fructose syrup.
Do you know who has been portrayed in paintings eating rich meats and drinking wine? Henry the eighth of England. Yes, the same Henry the eighth who was in the royal family of Prince Harry and Duchess Meghan Markle. Now, history dictates that when Henry the eighth was not preoccupied with beheading his wives, he had multiple gout attacks. In fact, it’s because of Henry the eighth that gout got the nickname the disease of kings.
What is the treatment of Gout? The most immediate treatment of gout is to take care of the pain and inflammation. How do you treat inflammation? With antiinflammatory drugs. So, your doctor can prescribe steroidcontaining antiinflammatories such as prednisone, which can be taken by mouth shortterm. Also, you can be given steroid injections inside of the affected joint. You can also be prescribed nonsteroidal antiinflammatory drugs, like Ibuprofen, Motrin, Advil, indomethacin. The key is to have a discussion with your physician. Find out if it is safe for you to take these antiinflammatories. If you are a patient with kidney disease or ulcers, or if you are predisposed to bleeding easily, then NSAIDs may be contraindicated in you.
Colchicine can be used to treat gout. Colchicine is a medication that helps to slow down or to stop inflammatory cells from migrating to that joint. So, colchicine is a drug that can be taken for the immediate treatment of gout. Colchicine does have certain side effects in certain patients. It can cause nausea, vomiting, and diarrhea.
So yes, steroids, NSAIDs, and colchicine can all be used to treat the inflammation and the pain of gout. And there’s something to be said about the pain that gout causes. Founding father Benjamin Franklin, who has been credited for inventing bifocals and discovering electricity also had gout. History reports that his pain from gout flares was so severe that he would actually have to miss many meetings about the Declaration of Independence.
Another part of the treatment of Gout is prevention or prophylaxis. You can prevent recurrent gout attacks by decreasing the amount of uric acid in the blood. This can be done through certain medications such as ULORIC, Allopurinol, probenecid. Again, a consultation with your physician will let you know which medication is right for you.

It is an awesome one of a kind product for getting rid of gout without the headache. Ive heard some interesting things about it and my friend after many years got great results with it.

  4. I had gout for 3 years now, i just notice everytime after i run or play sport, some sort of pain in my ankles or knee occurs, don’t know if it is still gout, if it is gout does it mean activity triggers gout?

  5. I had my first gout flare and it lasted 3 months . I literally just closed on the sale of my home and we were in the process of moving across the country…. I had to deal with the pain… now 4 months later I have modified my diet and have been on allopurinol, 100mg 1x a day… my problem is I just moved to southern CA and I know no one !!! I need a pcp, I was told to find and “internist”… anyone know
    Any great doctors in the DTLA area who would be able to help me???

  6. I'm not sure which was the thing that has helped me be free of gout flares for more than a year now and so I will list the things I've done. Before I do this let me add that I took the allopurinol for a while but every time I took the pill I'd get a pain on my right side and so I stopped it. Couldn't take pain meds because I have kidney desease. So… I started drinking alkaline water, drinking a glass of water with squeezed lemon juice in it (every morning about half hour before breakfast) and stopped eating so much bread, cookies, cake etc. Before I started getting gout I was already eating healthier and wasn't drinking sodas or alcohol. Personally I feel that it's been the lemon water that did it. So glad I finally got with the program and started using Google to do my own research. I have found a wealth of information on natural remedies for just about anything. Of course I've consulted with my kidney doctor and I also have to run anything I take thru the coumadin clinic. I take a blood thinner cause I had a heart valve replaced.

  7. I wish you were my Dr!! You seem soo nice and comforting! I have suffered with gout for many years..I had controlled it well the past few years by not eating pot roast or shell fish but I discovered these gummi vitamins..well they are real tasty…instead of 2 a day I was eating them like candy..like 10-12 a day…So I think I overdosed on vitamins and it made me urinate every half hour which caused me to get dehydrated…This is me self diagnosing as I have no health insurance to go to a Dr…I did find some Methylprednisolone and took a 6 day round exactly as the label said…it took away the pain for a few days but now it's back full force!! What s going on? My knee is killing me!! I started taking cherry extract capsules and Advil…5 at a time..I have real pain meds but I know they wont even work plus I dont like to take narcotics after going through hell to get off them once…I guess I should go to the ER and see what they will do…ugh

  8. Thank you for the clear information. Im lying in bed with a swollen toe full of pain looking for answers. Its my second attack. I am 46 and have been a sportsman until recently. I am still fit and muscular. I guess I will have to delete steaks from my diet and cut back on alcohol. P.S Franklyn did not discover elecricity. He underpinned the relationship between sparks and lightening

  9. Stop choking, good way to cure choking.lol seriously appreciate the inform.just left my doctor with this problem…before i went i drink a gallon of cranberry juice, i believe this help releaved most of the pain. I was prescribe some anti-inflammatory meds..

  10. You forgot to say diet and exercise can help cure gout. I guess if you stay away from alcohol and meat while exercising you should be good for the rest of your life.

  11. Drink water like you never have before. Cut down on sugar that’s what I did. I had gout in my knee that wouldn’t go. I cut down on red meat and drank slightly less alcohol but when I did I drank water as well. When I started to increase my water intake for 3 days my toilet habits didn’t change ie I dint wee more so I must of been dehydrated. After two weeks my gout went on it’s own. A month later the swelling went as well. Hope this helps at least one person because I now know how bloody painful gout is.

  13. I love how you explain things in such detail. Can you speak about a rare condition called Cyclic Vomiting Syndrome (CVS) My 12 year old son has suffered with this since he was about 2 or 3 but was just recently diagnosed after I did some research and discovered what he actually had and brought it to his doctors attention.

  16. Thanks doc. I have heard the same for 31 years now and suffered so badly usually a few times a year, but just plain annoying and limping for at least 6 times per year. I just want to know why Uric acid builds up in the body. That is the what every medical school is preaching and training doctors to write out a lifetime of prescriptions. So many individuals seem to know what their trigger foods are?
    Managing the Uric acid fluctuations is a very costly and painful affair to a seasoned gout sufferer. Not to mention the long term side effects.
    Not to mention the strain it puts on a family. Not to mention the emotional effects it has on some individuals. Oh, and yes kidney stones, you think a bad gout attack hurts? Wait till you get a kidney stone ! Whoah! I'm sure they will come up with a finding and a proper cure in about 20 years and I will be dead by then ….. God hasn't finished punishing me yet.

  17. i have both feet pain in toe area since last 5 wks which got worse when i walk, my uric acid is normal, no diabetes, cholesterol and cretinine thyroid xrays all normal, doctor advise me nuberol fort, xobix 7.5mg twice a day, and also zyloric 1/2 tablet daily but i dont know why he advise zyloric when my uric acid is normal, i am taking other two medicine since 02 days but not zyloric, plz advice whether i should start zyloric or not, plz plz help me out. waiting 4 your precious reply.

  18. Sounds like a real doctor. Prescribe more medication. How about prescribing a more natural remedy. That's what's wrong with the pharmaceutical companies today always wanting to prescribe more drugs.
