Gout, Pseudogout, and Joint Pain
The most common joint affected by gout is the first metatarsophalangeal joint. The most common joint affected by pseudogout is the knee joint. Gout and pseudogout both show a sudden onset of pain, redness, and swelling typically affecting a single joint in 80% of the cases. Gout and pseudogout are similar problems with different causes. Gout is caused by the buildup of uric acid and the deposit of uric acid crystals inside a joint. The best test to diagnose gout is with a joint fluid analysis. Gout crystals are needle shaped and negatively bifringent. When placed under polarized light, they will be yellow. 90% of patients suffering from gout are men between the ages of 4060 years old. Gout symptoms and signs include joint pain, swelling and arthritis. Patients with gout have periarticular erosions along with the formation of uric acid soft tissue masses in and around the joint which can be seen on xray. Soft tissue tophus deposition with periarticular erosions “punchout” lesions. The tophi occurs due to deposition of uric acid crystals. The tophus aspirate may look like tooth paste. The sudden attack of gout can be brought on by anything that increases the level of uric acid in the blood such as: dehydration, increased consumption of alcohol, eating large amounts of meat or seafood, or trauma/surgery. Other risk factors for gout are obesity, hypertension, and diuretics. Red meats, seafood, liquor, beer, all increase the risk of gout. Vegetables, wine, dairy products, and total proteins do not increase the risk of gout. Aspiration and analysis of the joint fluid is the best method for diagnosis. Elevate uric acid is not diagnostic. 80% of people with elevated uric acid will not get a gouty attack. There are blood tests such as white blood cell count, Creactive protein, sedimentation rate, and uric acid level that are helpful in supporting the diagnosis if elevated, but if these levels are normal, it cannot definitively rule out gout or pseudogout. Every time you aspirate a joint and you get synovial fluid, you need to analyze it for cell count differential, find out if you have crystals or not and send the fluid for culture and sensitivity if you suspect infection. It might be difficult to differentiate an acute gouty attach from acute septic arthritis. Patients with an acute gouty arthritis may not have an elevated serum uric acid level. A patient with acute gouty arthritis may present with symptoms and a clinical picture that is similar to septic arthritis. Aspirate the joint fluid, and the joint fluid will look like pus, but it could be gout. The incidence of gout and associated septic arthritis of a joint is low (about 1.5%). The incidence of septic arthritis will increase to 11% or more if the cell count is more than 50,000. We aspirate the joint (aspirate will look cloudy, like pus). We look for crystals and if there is crystals, then it is gout, but the presence of uric acid crystals does not exclude septic arthritis. We look at the cell count (will be high, 50,000 or more). The neutrophil count may be 80% or more (we think there is an infection in addition to gout or maybe gout alone). We need to culture the fluid. After we aspirate the fluid and send the fluid for culture, then we give the patient empiric intravenous antibiotics pending the culture result. Remember that gout and septic arthritis can occur together, but the incidence is low. The incidence will increase significantly if the cell count is more than 50,000. Pseudogout or chondrocalcinosis is the deposition of calcium pyrophosphate dihydrate crystals in the hyaline cartilage or fibrocartilage (CPPD). Pseudogout is a metabolic disease where calcium pyrophosphate dehydrate crystals (CPPD) are formed within the joint space. Pseudogout most often affects the knee, occurs more in older patients, and is a calcification of fibrocartilage (chondrocalcinosis). Pseudogout crystals are rhomboid shaped and positively birefringent. Crystals will be blue when placed under polarized light. Associated conditions include hyperparathyroidism, rheumatoid arthritis, and gout. Aspirate to see if it is pseudogout or infection, because you do not want to inject the knee with steroids when there is an infection. You need to look for the rhomboid crystals of pseudogout. Xrays in pseudogout will show thin calcification in the articular cartilage or menisci. Calcifications of the synovium, tendon, and ligaments can also occur. Acute gout can be treated with indomethacin or colchicine if the patient cannot tolerate NSAIDs. Colchicine inhibits the inflammatory mediators and is indicated if the patient cannot tolerate indomethacin. Chronic gout can be treated with allopurinol to prevent buildup of uric acid.

This weeks video idea came to me after a patient asked me what they can do, to help prevent gout attacks.

Gout is in fact one of the most common forms of arthritis. We all know someone who suffers from gout so please help spread the word on this video so more people can learn how to prevent an attack.

WHAT CAUSES GOUT

Gout is caused by having too much of the chemical, uric acid, in your bloodstream. Uric acid is the waste product created when the body breaks down purines (a type of protein found in many foods and all of your cells).

Most of the uric acid is passed out with the urine and some from the gut with the stools (faeces). In people with gout the amount of uric acid in the blood builds up. From time to time the level may become too high and tiny gritlike crystals of uric acid may form. The crystals typically collect in a joint. The crystals irritate the tissues in the joint to cause inflammation, swelling and pain a gout attack.

HOW TO PREVENT GOUT ATTACKS

Lifestyle factors may reduce the risk of having gout attacks. These include losing weight (if overweight), eating a healthy diet and not drinking much alcohol or sugarsweetened soft drinks. Feel free to watch my video for more detail.

SIGNS & SYMPTOMS OF GOUT

Any joint can be affected by gout, but it usually affects joints towards the ends of the limbs, such as the toes, ankles, knees and fingers. Signs and symptoms of gout include:

• Severe pain in one or more joints
• The joint feeling hot and very tender
• Swelling in and around the affected joint
• Red, shiny skin over the affected joint

Symptoms develop rapidly over a few hours and typically last three to 10 days. After this time the pain should pass and the joint should return to normal.

Almost everyone with gout will experience further attacks at some point, usually within a year.

See your GP if you suspect you have gout and it hasn’t been previously diagnosed, particularly if the pain keeps getting worse and you also have a high temperature (fever).

It’s important that a diagnosis is confirmed because other conditions that require urgent treatment, such as an infected joint, can sometimes cause similar symptoms.

Gout is caused by urate crystals being deposited in your joint. The urate crystals are produced when our body has too much uric acid in it. Dietary changes are critical in maintaining proper health and preventing a gout attack. Organ meats are also very important to avoid. Organ meats, such as liver, contain purines, which when they break down to uric acid can overload our body.

Certain foods can actually decrease the amount of uric acid in our body. Coffee, believe it or not, can actually decrease the amount of our uric acid in our body. Fruits that contain vitamin C can also decrease the uric acid. For years, the old wives’ tale was: “Eat a cherry a day and you can keep the gout away.” It has actually been proven to be true. Some of the darker berries can also decrease the amount of uric acid in your body.

You’ve been modifying your diet, and you still get a gout attack. What do you do? First, take some antiinflammatories and call your doctor. We have very good medications in managing an acute gout attack. These would include oral antiinflammatories, a medication called Colchicine, and in some cases steroids like Prednisone. There are also great medications in preventing future attacks. Medications can control your level of uric acid, or actually help you excrete the uric acid more effectively.

Gout needs to be treated. Do not ignore it. The consequences of recurrent gout attacks include arthritis, tophaceous deposits, which are accumulations of little nodules around your joints, or actually even kidney stones. Seek medical attention should you have any of these symptoms. And as always, if you have redness, swelling, and fever, go to your local emergency room. It’s probably not a gout attack, but a more serious infection.

  1. Good video. However, telling people to believe that pseudo gout can be (just) in the knee is a little misleading! I have recently had my parathyroid gland removed due to Hyperparathyroid diagnosis. I have been hit with pseudo gout for the second time. Both times in my feet triggered by minor injuries. Not ankle, not knee.

Hi I've had this since I was 36 in both knees I'm 43 now can't do bugger all all the doctor and rheumatologist give me as anti-inflammatories I live in New Zealand can't cycle anymore because my knee swells up struggle to exercise and walk ,they extracted fluid from my knee and said I had pseudogout . I find if I drink a lot of alcohol it inflames my knees and wrists so I've been doing my best to stop please help

  3. I'm currently studying my 4th year of medicine from home. Teachers basically send a tone of papers and documents for us to read, it's ridiculous.
